健忘症是指大脑存储或检索记忆的方式存在障碍,因而失去记忆信息、事实和经历的能力。健忘症大多数可能是暂时的,医学上称之为暂时性记忆障碍,但少数也可能是永久的。

健忘症是指大脑存储或检索记忆的方式存在障碍,因而失去记忆信息、事实和经历的能力。健忘症大多数可能是暂时的,医学上称之为暂时性记忆障碍,但少数也可能是永久的。健忘症一般分为如下几种类别:
- 顺行遗忘症:指不能回忆几分钟前发生和看见的信息或事件情况,即无法学习新信息并形成新的记忆。
- 逆行遗忘症:指对回忆以前的信息、或发生事件的细节出现困难。
- 短暂性完全遗忘:暂时的失忆症在短期记忆的影响。
病因
大多数记忆丧失或无法创造新的记忆是由大脑受损造成的。记忆是由大脑的几个部分之间复杂的相互作用而产生的。失忆症可能是由于形成边缘系统的大脑结构受损所致,它们控制情绪和记忆。这些结构包括位于大脑中心深处的丘脑和位于大脑颞叶内的海马结构。1.由脑损伤或创伤引起的健忘症被称为神经性遗忘,可能原因包括如下:
- 脑卒中
- 脑炎,由下列原因引起:
- 单纯疱疹病毒等

风险因素

有助于防控健忘症的营养和草本主要包括如下:
1.胆碱和丝氨酸磷脂:
胆碱和磷脂酰丝氨酸是支持参与记忆获取和回忆的神经元结构和功能的脂质成分。胆碱通过两种不同的机制在记忆的神经元过程中发挥着重要作用。首先,胆碱是神经递质-乙酰胆碱的前体,乙酰胆碱对参与记忆的神经元信号传导很重要1。因此,提高乙酰胆碱水平的药物是治疗阿尔茨海默病的重要组成部分。其次,大脑中的神经元具有高水平的磷脂酰胆碱,这是一种存在于细胞膜中的脂质。据认为,神经元中磷脂酰胆碱水平的降低可能导致记忆丧失和其他形式的认知能力下降2。
在记忆丧失的动物模型中,补充胆碱通过改善记忆巩固过程来缓解记忆障碍3。人体试验也取得了有希望的结果。补充甘磷酰胆碱(也称为α-GPC,一种胆碱形式)已被发现可以提高记忆和注意力。在年轻、健康的个体中,α-GPC已被证明可以预防药物诱导的健忘症4。已经进行了超过23项关于α-GPC的临床试验。观察到的一些好处包括提高注意力、记忆力、集中思维和认知能力5。还发现这种化合物可以减少中风引起的记忆力丧失和其他神经损伤迹象6,7。另一项研究发现,胆碱摄入量较高的人在记忆测试中表现更好,例如测量视觉和言语记忆的测试,以及其他认知功能测试1。
丝氨酸磷脂是另一种对神经元健康很重要的脂质,是研究最广泛的记忆障碍化合物之一2。动物研究发现丝氨酸磷脂减轻了药物诱导的健忘症8,9。此外,多项研究表明,补充丝氨酸磷脂,无论是单独补充还是与其他脂肪复合,都能改善有记忆问题的老年人的记忆10-12。具体而言,丝氨酸磷脂改善了对信息的言语回忆,并可能改善有记忆障碍但没有痴呆迹象的老年人的认知缺陷10,12。磷脂酰丝氨酸也可能改善注意缺陷多动症患者的记忆力13。除了保持神经元健康外,丝氨酸磷脂还可以通过提高大脑中葡萄糖(有助于神经元的能量消耗)和乙酰胆碱的水平来改善记忆2。另一项临床研究发现,丝氨酸磷脂可能增强银杏叶的作用,银杏叶是一种以有益大脑健康而闻名的植物14。
2.维生素B族:
B族维生素,包括叶酸、维生素B1、维生素B6和维生素B12,也可能在预防健忘症方面发挥重要作用。维生素B12在维持神经系统健康方面的作用尤其明确,因为缺乏维生素B12可能会对外周神经系统造成损害15。除了维生素B12,叶酸和维生素B6对认知功能的重要性也成为人们关注的焦点,这些维生素都是降低大脑中同型半胱氨酸水平所必需的;高同型半胱氨酸(高血同)可导致血管功能障碍16,17。同型半胱氨酸水平往往随着年龄的增长而升高,并与阿尔茨海默病风险的增加有关15,18。
高同型半胱氨酸除了是中风的危险因素外,对阿尔茨海默病患者和健康人的神经元都有直接毒性。B族维生素有助于预防健忘症和其他形式的认知能力下降的方式之一是降低高血同水平。它们还可能有助于合成大脑正常工作所需的不同神经递质17。几种维生素B12相关标志物的低水平也与脑容量低和认知功能下降有关15,19。
一项研究报告称,连续2年补充叶酸、维生素B12和维生素B6可以减缓认知能力下降并改善临床状况20,而其他研究报告称它们也可以减缓脑萎缩15,21。确保摄入足够的硫胺素(维生素B1)是预防健忘症的一个重要考虑因素,因为缺乏硫胺素会导致Korsakoff综合征,其症状就是健忘症22。
3.乙酰L肉碱:

4.辅酶Q10和吡咯喹啉醌:

此外,在人类中,补充EPA和DHA已被证明可以改善工作记忆47。
6.镁:
大脑和脊椎周围的液体(称为脑脊液)中的镁含量高于血液中的镁水平。这表明镁在中枢神经系统中起着重要作用48。在动物模型中,镁缺乏的饮食增加了脑损伤后的记忆缺陷,急性脑损伤后给予镁显著减少了组织损伤并改善了行为结果。此外,一项研究表明,遭受创伤性脑损伤并接受低镁饮食的动物死亡率更高,神经系统预后更差49。证据还表明,轻度至中度阿尔茨海默病患者血液中的镁浓度通常会降低50。

7.银杏叶:

8.南非醉茄:

9.石杉碱甲:

研究还表明,在口服给药8-24周后,每天300-500mcg的石杉碱甲改善了阿尔茨海默病患者的精神状态,并且耐受性良好67。它还可能有益于因血管疾病而导致认知缺陷的人68。此外,石杉碱甲在动物模型中显示可促进海马内神经元的生长69。
10.长春西汀:
长春西汀是长春花植物的衍生物,可能有助于治疗由于潜在的神经问题引起的健忘症。除了具有神经保护作用外,长春西汀还可以通过改善海马体和大脑其他控制记忆部分的神经元的功能来增强记忆70,71。动物和细胞培养研究表明,长春西汀可以改善记忆,保护神经元免受各种不同类型的损伤70-73。注意:怀孕或可能怀孕的妇女不应使用长春西汀。
11.蓝莓:
蓝莓中特别富含类黄酮的化合物,类黄酮还可以增强大脑的血液流动并优化神经功能74。许多动物研究发现,用蓝莓补充饮食可以增强记忆力75-77。蓝莓补充剂的作用可能会调节海马体中的一些生化信号通路,而海马体与记忆密切相关78。一项针对老年人的小型研究还发现,每天食用蓝莓汁12周可以改善记忆力79。

医疗干预
目前,没有针对健忘症的治疗方法。
- 治疗暂时性遗忘症将集中在病因上。有些遗忘症只会持续几天,只需要观察直到症状消失为止。
- 长期失忆症:需要职业疗法和新技能来帮助管理日常活动。职业疗法有助于使用记忆训练法来更好地管理记忆丧失。技术性工具、笔记本、或提醒物如照片这样的也可能有助于管理记忆间隙。
预防
由于对大脑的损害可能是健忘症的根本原因,因此采取措施尽量减少脑损伤如:- 避免过量饮酒
- 骑车时戴头盔,驾驶时戴安全带。
- 尽快治疗任何感染,使其无法扩散到大脑。
- 如果有任何症状表明中风或脑动脉瘤,如严重的头痛或单侧麻木或瘫痪,应立即就医。
